What is an Ultrasound-Guided BBL?
An ultrasound-guided Brazilian Butt Lift (BBL) is a fat transfer buttock enhancement where Chicago BBL surgeons Dr. Anh-Tuan Truong and Dr. Kevin Lin uses real-time imaging technology during the fat injection. Rather than injecting fat “blindly,”, the ultrasound technology allows your surgeon to see exactly where the fat is being placed, and confirm the cannula tip that is used to inject the fat stays in the fatty layer above the gluteal muscle.

Quick takeaways
- Ultrasound helps us see cannula depth during fat transfer.
- The goal is to keep fat placement above the muscle in the subcutaneous space.
- Better visualization can support safer, more controlled technique, but it does not eliminate risk.
- Results depend on your anatomy, skin quality, donor fat, and healing.

Who this is for (and who it is not)
You may be a good candidate if you:
- Want a natural, proportional change (not an exaggerated look)
- Have enough donor fat for transfer
- Can commit to recovery, including activity limits and compression for at least 4 weeks
This may not be the right procedure if you:
- Want the biggest possible result regardless of anatomy
- Are shopping primarily by price
- Are not able to follow post-op instructions closely
- Have medical risk factors that increase surgical risk (we review this carefully at consult)
If someone is not a good fit for a Brazilian Butt Lift, we can provide other alternatives like Sculptra buttock injections.

How is an ultrasound-guided BBL different?
During the fat transfer portion of a BBL, the ultrasound gives us real-time imaging so we can confirm the cannula and fat placement is injected into the subcutaneous fat layer.
The ultrasound-guided BBL procedure: what to expect
1) Consultation and planning
We start with your goals and then match them to anatomy. Our surgeons look at factors like:
- Your frame and proportions
- Skin quality
- Donor fat availability
- Where you want volume versus contour
A common misconception is that BBL is just “add fat, get curves.” In reality, the liposuction contour and the placement plan matter just as much as the volume.
2) Fat harvesting (liposuction)
Fat is removed from donor areas (commonly abdomen, flanks, back), then purified and prepared for transfer. If you need additional fat because you have a low BMI, your surgeon may also recommend harvesting fat from your arms, thighs or axillary (armpit) areas.
3) Precision fat transfer with ultrasound guidance
During injection, ultrasound helps confirm the cannula is where we want it so fat placement stays subcutaneous. The injection is done in a controlled pattern to build shape gradually, with attention to symmetry.
4) Recovery and results
Most patients can return to light activities in about 1-2 weeks, but full recovery takes longer to allow your body to take its new shape. Final contour continues to settle over the next 3-6 months as swelling improves and the transferred fat stabilizes.

Is an ultrasound-guided BBL safer?
Ultrasound can improve safety by adding real-time visualization during injection. It helps confirm cannula position and intended placement. It does not remove risk entirely.
Does ultrasound-guided mean fat cannot go into the muscle?
The goal is subcutaneous-only placement. Ultrasound helps confirm plane and depth during injection. Your surgeon’s experience and technique are also extremely important.
How long does it take to see final results?
You’ll see change right away with your final results seen over the next 3-6 months.
Will all the fat stay?
Not all the fat will survive. The fat retention varies person to person. Following recovery instructions and maintaining stable weight helps support your long-term result.
When can I go back to work?
Many patients return to non-physical work in 1 week. You’ll be sore but you should be up and walking around the next day.
